Please print out this form and then mail or fax the information to our Hong Kong office. The fax number is (852) 2815-5032.   Please note, credit card companies require that we have the actual signature of the credit card holder in order to process the payment.  That is why we cannot have you simply e-mail this form back to us.  We need the physical signature or a faxed copy of it.  The one way you can e-mail this form would be if you first print it out, then sign the paper copy, scan it back into your computer, and then e-mail it to us as an attachment to our address info@asiarisk.com.  We apologize for this inconvenience, but it seems to be the only way we can accommodate the credit card companies.  Since we are in Hong Kong, all US dollar credit card payments will be converted into Hong Kong dollars at the rate of HK$7.8:US$1.
